I believe the issue is with the os or driver since it doesn't work at 4k in windows 10 in bootcamp but does work in mac os x.

4k at 60hz from my mid 2015 macbook pro!

Worked great on my 2015 mbp to my lg ultrawide monitor.

The mini display port to dvi adapters with a dvi cable don't work at the native resolution of ultra-wide monitors.

If you have a high-end machine (4-5k ultra) it really does make a difference as far as a i can tell.
