Swivel swivels itself off of the bolt that secures it. Rotation loosens the bolt further until the whole thing drops to the floor.

Not sure how anyone is using this without securing the central bolt. Get a locking bolt and perhaps some loctite.

You must drill the right size pilot hole some have said the bolts break easy i didnt have a problem, or replace bolts with stronger ones.
