Seemingly at random a settlement would show it was unhappy because it suddenly had no beds water or defences

The only workaround that seems to reduce the problem is to avoid fast travel and walk in and out of settlements but this isn’t always practical and doesn’t always work

A silly design flaw in the game is to automatically eject a fusion core when you don’t notice you’ve run out of fragmentation grenades
