I didnt use the strap at all because i have a pain upper back most but somwtime lower part more so depends of the day i move around to use it

I think the thickness is not distributed well its thick in the lower back and thin in top of the lower neck which cause me pain hence there is a gap and no support for your lower neck the design of this lumbar support pillow should be change to consider to put more cushion in that area
